Oh sweet jesus. I worked once at a fairly well know large place and all their apps did in fact have dedicated dev/test/stage/prod environments, however there seemed to often be no relationship between the code states, Stage would have an new version than test and sometimes no one would have touched dev in years.

Some of these sites ran basically on script files so there wasn't a built app, few if any of the sites had source control, people would literally edit files in prod.
